1st grade - 5th grade
6th grade - 8th grade
In addition to the above required forms, please include:
-Copy of Birth Certificate
-Copy of Social Security Card
-Immunization Records (may be faxed directly from doctor)
-Baptism Certificate (if Catholic)
-Most recent Report Card (if available for grades Pre-K - 1st)
-Standardized test results (if available for grades Pre-K - 1st)
-Application Fee
